Complete the three fields below to receive claim status.

First enter your provider Tax ID Number. Then, select a Medicare Number (HIC #) and last name of any patient for which we've paid a claim in the last two years. These two fields must be the same person.

Answer

Providers may use our automated voice system, our provider web portal or they may submit their request in writing. The web portal is very user friendly and offers many options for claim review and submission.
